A. Introduction

 

SCOPE is Socorro County’s community health council; it presently receives funding from the New Mexico Department of Health.  SCOPE’s fiscal agent is Socorro Mental Health, Inc. situated on Hwy 60 in Socorro, NM.  SCOPE employs a Coordinator to organize and oversee contracted work on health issues and priorities determined by the health council and consistent with those of the NM Department of Health.  The SCOPE membership meets on the third Thursday of each month. SCOPE Definition of Health

Health does not just mean the physical wellbeing of the individual but refers to the social, economic, emotional, cultural and spiritual wellbeing of the community. Health is not merely the absence of disease or infirmity.

RFP Purpose

The purpose of the SCOPE 2006-2007 Substance Abuse Priority Project, request for proposals (RFP) is to provide mini-grant funding to county entities for the purpose of expanding or initiating activities in addressing substance abuse.  This is one of five health priorities that SCOPE has identified for Socorro County, New Mexico.  This priority was determined by reviewing and analyzing County health data.  The Mayor’s Drug Task Force’s aim is to increase the age of first use of alcohol, tobacco, and other drugs.  The proposals that address this aim will have first priority.  Some examples of potential activities for this priority are:

·          Peer or adult mentoring program

·          Training for those who will be presenting or teaching material to a targeted population.

 

Funding Availability and Authorization

The total funding under this RFP 2007-1 is approximately $4,500.  The awarded amount will be adjusted based upon activities proposed by potential contractors.  Authority of funds allocated will be left to the discretion of a committee composed of Mayor’s Drug Task Force members along with the discretion of the Fiscal Agent, Socorro Mental Health.

RFP Allowable Activities

This project is to support innovative, non-duplicated services in Socorro County, New Mexico, which will address the substance abuse health priority identified by SCOPE.  Each project must include an evidence-based evaluation (see section B2e for guidance on evaluation).  Entities submitting proposals must be a community partner member, in good standing, of the SCOPE health council.  Current non-member entities wishing to submit proposals are welcome to join SCOPE prior to the submission deadline as long as all bylaws requirements for membership are met.  There is no fee to join SCOPE; requirements for Community Partners are to attend a minimum of three meetings and participate in setting SCOPE priorities and plans.

 

A representative of the agency submitting the proposal must agree to attend monthly Mayor’s Drug Task Force meetings and submit monthly reports of their activities.  A reporting format will be jointly developed at contract negotiation and must be submitted to ensure compliance and final payment. This will ensure collaboration and promote success of their proposal project.  All proposal activities must be completed by June 15, 2007 and final reports will be due to SCOPE by June 30th along with an oral presentation.

2. Proposal Format

All proposals must be submitted in MS Word using number 12 Arial font type, with a one inch margin on all sides.  The proposal must include the sections outlined below.  Incomplete proposals and proposals not received by the specified time will not be reviewed.

a. Contractor - Proposal Contractor (Requestor) Assurances

1). Organizational Chart with a narrative description.

2). Job Descriptions of personnel who will carry out the activities to be supported.

3). Coordination and Linkages – Attach letters of support or memoranda of understanding that demonstrate cooperation among those involved in the activity for which funding is being requested.

4). Proof of Liability Insurance, if needed

5). Articles of Incorporation.

6). Equal Employment Opportunity (EEO)/Affirmative Action Plan if applicable.

b. Activity – One page describing the proposed activity, the population to be served, the timing of the activity, a description of the activity, the objective to be achieved by the activity.

c. Budget – complete the provided template for your budget including the line items and the budget narrative justification.  (Budget Template can be found in Appendix A)

d. Capability – One page describing Contractor’s (requestor’s) applicable experience, ability to manage grant funding, and ability to conduct the proposed activity.

e. Evaluation – One page description of the methods that will be used to evaluate the activity’s effectiveness in addressing the Socorro County health priority of substance abuse.  The evaluation plan must include one or more measurable outcome objective(s), measurable process objectives, and list/describe the tools that will be used to collect those data.

An example of a measurable outcome objective is: “We will see a 20% increase in the number of people who say that marijuana use by youth is unacceptable.”

An example of a measurable process objective is: “We will provide information on the harmful effects of marijuana use to 200 people.”

An example of a tool to measure the outcome is: “We will use the 5 question pre- and post-test provided by the developer of our selected brief intervention.”  An example of a tool to measure the above process objective is:  “We will keep a sign in sheet of all people who receive the brief intervention.”

C. Basis of Award

Provide as much detail as you can within the required page limit. Proposal evaluation will be as follows:

 

·        Active community Partner of SCOPE – required

·        Addressing the SCOPE priority of substance abuse – required

·        Merit of proposed activity  (30 points)

·        Targeted population / participants (15 points)

·        Capability of contractor (15 points)

·        Evaluation plan (20 points)

·        Budget and budget explanation (20 points)

In accordance with all State and Federal regulations, including the HIPPA Privacy Rule, all grant recipients will follow the policy of SCOPE to maintain the security, integrity, and confidentiality of data and other information pertaining to the clients served by grantees of this SCOPE 2006-2007 grant. This includes, but is not limited to, documentation of activities, and the collection, storage, and retrieval of data.

D. Notifications

All applicants will be notified of proposal status via US postal service no later than Tuesday, February 6, 2006.

E. Payment Schedule

Successful proposals will receive funding on the following schedule:  Fifty percent of the total grant amount will be dispersed at the time the contract negotiation is completed; and the remaining amount will be paid after all contracted work is completed, the evaluation report and all monthly reports (see section F for further explanation) are submitted.
